Your First Visit

Prior to your first appointment, please fill out our New Patient Form* online.

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

Your initial exam will be is holistic in nature; we look at your oral health in context of your overall health. It's our goal to develop a dental health plan that supports the health of your oral structures and your body as a whole. To this end, we will compile a thorough file of personal information, including your medical and dental history.

Your dentist will then complete a thorough dental exam, including an examination of your bite, teeth, gums and jaw, an oral cancer screening, and any x-rays that may be required.

Treatment Plan

Your dentist will develop a long-term dental treatment plan for you based on your examination and dental history. There will also be time to ask any questions you may have about your dental health and treatment options.

• How can I book an appointment?

  Please contact our office during our regular business hours to book an appointment at Manor Park Dental.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so we can offer another patient the space in our schedule.

• Do you provide services in any languages other than English?

  Yes, our office is able to provide dental services in several languages including English, French, Spanish, and Persian.

  If you have a language preference, please advise us when booking your appointment.

• Does your clinic have parking?

  Yes, free street parking is available for all our patients. We are also easily accessible by public transit.
